Services We Provide

Serving the communities of Oakville, Milton, Halton Hills, Mississauga and South Etobicoke, we work on behalf of our health system partners to:

  • Assess client eligibility for these programs based on standardized criteria
  • Prioritize incoming referrals
  • Assign clients the most appropriate service provider to meet their needs
  • Track and trend the demand for services
  • Maintain a centralized wait list for services.

Generating peace of mind with one phone call

With one phone call a Central Registry team member will help determine your eligibility for these programs and connect you with the care you need or refer you to alternative resources that may better meet your needs.

We are your one-stop point of access in the Mississauga Halton Local Health Integration Network (MH LHIN) for:

           
Supports for Daily Living is a model for community-based assisted living that provides high risk seniors with access to pre-scheduled and/or urgent/on-call personal support, seven days a week, 365 days a year.     The Caregiver ReCharge Services provides temporary relief for caregivers who are experiencing high levels of stress or burnout as a result of their care giving activities.     Short Stay Respite occurs when the care recipient
temporarily stays in an out-of-home environment in order to provide relief to the caregiver.
    Food advice and Food Delivery programs are available to support individuals in the community to access nourishing meals.
