How to make a referral

We welcome referrals from a variety of referring sources including hospitals, community partners, healthcare professionals as well as self or family referrals. Our Central Registry team is available to help you with questions you may have about Caregiver Recharge services and making a referral.

We help caregivers continue caring for their loves ones

The Caregiver ReCharge Services provides flexible, worry-free, in-home relief for caregivers who can use a much-needed break from caring for the day-to-day needs of a loved one with high needs. Developed by the Mississauga Halton Local Health Integration Network (LHIN), the Caregiver ReCharge Services serves the communities of Oakville, Milton, Halton Hills, Mississauga and South Etobicoke. We are a collaborative community service offered by the Alzheimer Society of Peel, Nucleus Independent Living, Links2Care and our healthcare partners, AbleLiving and Home Instead.
