How to make a referral

We welcome referrals from a variety of referring sources including hospitals, community partners, healthcare professionals as well as self or family referrals. Our Central Registry team is available to help you with questions you may have about Caregiver Recharge services and making a referral.

Services We Provide

Reliable temporary in-home support for your loved one

When you need a well-earned break, whether it’s for a few hours, a day, or a weekend, you can be confident that the care of your loved one is in excellent hands while you are away from home.

Service features

The Caregiver ReCharge Services offer:

  • A customized service plan to meet your unique needs as a caregiver
  • Skilled substitute in-home care giving by trained staff
  • In-home support services available any time of day throughout a 24-hour period

Scope of services

Our in-home support services include:

Activities of Daily Living

  • Movement and transfers
  • Dressing and undressing
  • Personal hygiene, bathing and going to the bathroom

Instrumental Activities of Daily Living

  • Preparing meals, help with eating
  • Medication reminders
  • Homemaking (i.e. light housekeeping, laundry)

Companionship Support/Safety and Risk Monitoring

  • Some recreational activities (i.e. playing cards, reading, games, etc.)
  • Redirection, prompting, cueing
  • Safety checks, helping to prevent wandering
