The Supports for Daily Living program was awarded the inaugural Minister’s Medal Honouring Excellence in Health, Quality and Safety in November 2013 for providing community resources to seniors discharged from hospital so they can recover at home rather than a long-term care facility. The Minister’s Medal recognizes the collaborative efforts of Ontario’s health service providers in improving care.
The Supports for Daily Living program was recognized with a prestigious 3M National Award for Health Care Quality at the National Conference of Canadian Health Care Leaders in June 2011.
The 3M Health Care Quality Team Award in the non-acute category recognizes innovation, quality and teamwork. The pioneering Supports for Daily Living initiative has had proven success in reducing hospital stays for seniors, reducing unnecessary ED visits and developing alternatives in the community to avoid long-term care home placement for seniors who don’t need this level of care.
