How to make a referral

We welcome referrals from a variety of referring sources including hospitals, community partners, healthcare professionals as well as self or family referrals. Our Central Registry team is available to help you with questions you may have about Supports for Daily Living services and making a referral.

Information for Clients/Families

Who is eligible for services

Supports for Daily Living is designed to support high risk seniors, aged 65+, who have complex needs, but who are medically stable. These services are currently available to individuals who:

  • Live in their own private home, or within a residential setting such as an apartment building
  • Demonstrate a need for daily access to personal support services throughout a 24 hour period (with or without homemaking services)
  • Are able to direct their own care or have a substitute decision maker or a live-in caregiver to direct care
  • Are able to communicate their needs (with or without aides)
  • Pose no risk to themselves or others.

Cost for services

Supports for Daily Living services are funded by the Mississauga Halton Local Health Integration Network (MH LHIN). There is no cost to eligible seniors for the service.


What to expect during the referral process

The following are the steps you can expect during the referral process:

  • Once Central Registry receives your referral you will be contacted to schedule an eligability assessment with a Central Registry Coordinator.
  • The Central Registry Coordinator will conduct an assessment to determine eligibility for the program and gather the information needed.
  • If you are eligible for the program, your file will be added to a waitlist until there is a vacancy available in the Supports for Daily Living system.
  • While on the waitlist, a Central Registry Coordinator will contact you periodically to update your file.
  • Once there is a vacancy in the Supports for Daily Living system, your file will be sent to an approved Supports for Daily Living provider to develop a customized service plan with you. Once a service plan is in place, your Supports for Daily Living services will begin.

If you are not eligible for this service our team will help facilitate referrals to more appropriate agencies on your behalf to find a program that best fits your care needs.
