How to make a referral

We welcome referrals from a variety of referring sources including hospitals, community partners, healthcare professionals as well as self or family referrals. Our Central Registry team is available to help you with questions you may have about Supports for Daily Living services and making a referral.

We deliver high quality assisted living services you can trust

Supports for Daily Living provides a range of assisted living services to support high risk seniors to continue living independently in the comfort of their own homes, including personal support, essential homemaking services and safety and reassurance checks. Services are scheduled at multiple times throughout the day or overnight according to your needs and preferences. Each scheduled time with you is customized to your needs, depending on the level of support needed and the activity involved.

Service features

  • 24-hour urgent/on-call response
  • 24-hour pre-scheduled services, 7 days a week, 365 days a year
  • Safety checks (via regular contact through intermittent visits)
  • Non-medical services
  • Skilled personal support workers (PSWs)

Examples of urgent/on-call responses include unexpected situations that require prompt attention (i.e. a fall, vomiting, bowel/bladder incontinence, etc.).

Scope of services

Supports for Daily Living provides personal support services that may include personal hygiene activities (washing, bathing, hair care), personal routine activities of daily living (dressing/undressing, toileting, help with eating), and essential homemaking services

  • Help with pre-measured medications
  • Medication reminders
  • Mouth care
  • Preventative skin care

  • Transferring/positioning
  • Washing/bathing
  • Essential   homemaking services

*Please note: the need for personal support services must exceed the need for essential homemaking services. Homemaking services can be provided as it relates to the care being provided.

You may be eligible for up to 1.5 hours of personal support each day, seven days a week, 365 days a year.

Types of services

Supports for Daily Living uses three different models to deliver services:

Designated Buildings (Hub)

There are several designated buildings throughout the Mississauga Halton Local Health Integration Network (MH LHIN) that provide Supports for Daily Living services. Personal Support Workers (PSWs) are located onsite and deliver services to eligible seniors in their individual units, apartments or condos within the building.

Designated Neighbourhoods (Hub and Spoke)

In designated neighbourhoods, the Supports for Daily Living service provider has an onsite office in a building that services seniors who live there, but also provides services to eligible seniors who live in neighbouring apartment buildings, town homes, condos or homes within close proximity to the building.


The Mobile service focuses on getting high risk seniors who are being discharged from hospital home quickly regardless of where they live. Supports for Daily Living service providers travel throughout the region to deliver services until seniors are stabilized at home, then transition seniors to other community partners including other Supports for Daily Living service providers, the Community Care Access Centre (CCAC), or a combination of services that can address a senior’s needs.

How we differ from regular home care

Regular home care services are coordinated through the Mississauga Halton Community Care Access Centre (MH CCAC). While CCAC home care services and Supports for Daily Living both support seniors in the community with high needs, Supports for Daily Living services are designed for seniors who need shorter, more frequent visits to help prevent premature placement in long-term care.
