We are excited to share an upcoming Education Series session, presented in partnership with Summerville Family Health Team and Peel Senior Link: Transitioning Home from Hospital, led by Central Registry, a division of Nucleus Independent Living.


This session is designed to support older adults and caregivers by providing the knowledge and confidence needed for a safe and smooth transition from hospital to home. Key topics will include preparation, home safety, and continuity of care.
